All Centers of Excellence (CoE) share a common set of services, including primary HIV medical care, case management, outpatient mental health, outpatient substance abuse, and treatment advocacy. Each CoE additionally has specialty services from other funding sources. CoE vary in terms of size, location and expertise in the needs of specific populations. Any qualifying client may receive services in the CoE of his / her choice.
The San Francisco Centers of Excellence network forges a new type of “safety net” for severe need and special populations. Currently, HHS funds seven CoE:
Chronic Care HIV/AIDS Multidisciplinary Program (CCHAMP) This CoE offers services to men who have sex with men (MSM); Latino, African American and transgender women; immigrants and undocumented Spanish-speaking persons, with locations in the Mission/Potrero Hill, South of Market, Upper Van Ness and Castro districts. The lead agency is the University of California in San Francisco.
Forensic AIDS Project Center of Excellence This CoE focuses on the coordination of care for HIV-positive incarcerated people both in jail and post-release and serves five San Francisco County jails with an average daily census of 2,200 prisoners. The lead agency is the San Francisco Department of Public Health.
Mission Center of Excellence This CoE focuses on services for Latino/Latina populations, including monolingual Spanish speakers, immigrants and undocumented persons. The CoE is located in the Mission district.
Native American Center of Excellence This CoE focuses on Native Americans and Alaska Natives, including male, female and transgender persons. The CoE is located in the Mission District, where medical care is provided, with additional services in Portero Hill.
Southeast Partnership for Health This CoE focuses on underserved and severe need African American populations. The CoE is located in the Bayview / Hunters Point and Western Addition.
Tenderloin Area Center of Excellence This CoE focuses on homeless, marginally housed persons; active substance users; transgender persons; Asian / Pacific Islander groups; and prison populations. The CoE is located in the Tenderloin. The collaborating agencies are Tenderloin Health (TLH, lead agency), Tom Waddell Health Center (TWHC), Asian Pacific-Islander Wellness Center (API), and the Harm Reduction Therapy Center (HRTC). These following sites offer medical services: Tenderlion Health - 187 Golden Gate Avenue. Tom Waddell Health Center - 50 Lech Walesa Street (between Polk/Van Ness & Grove/Hayes) Asian Pacific-Islander Wellness Center - 730 Polk Street, 4th Floor Intake by walk-in at TWHC is on Tuesdays starting at 1:30pm. Intake by walk-in at API and TLH is on Monday-Friday, 9am – 12noon and 1pm - 5pm. Women’s Center of Excellence This CoE focuses on underserved and severe need women. The CoE offers medical care in the Mission and Parnassus Districts, with additional services available in Upper Van Ness and the Western Addition.
